FAMILY SUPPORT FUND REQUEST PORTAL
Before submitting a FSF request:
- Review the Family Support Funds Purpose and Process document prior to each submission. This document and the request form are for internal use only.
- Refer to this step-by-step manual on how to make and submit a request.
- Contact your program leads or family engagement coach to discuss the family's situation.
- Receive approval from the program lead or family engagement coach prior to submission. The form approver must have a PSESD email address.
- Include all information on the request form rather than verbally or through separate emails.
- Forms are to be completed by staff (i.e., requester) in partnership with the family.
- On behalf of the family, the requester will be responsible for submitting all follow-up documentation identifying how funds were spent or allocated, unless the request is for a specific item or gift card. Plan to attach written or digital documentation within two weeks after receiving funds.
